Learning from patient safety events can only properly occur if those events are accurately, objectively and comprehensively reported, investigated and disseminated. The skills required to do this effectively are mandatory for any practitioner committed to safe practice. In this session, we look at the key elements of creating an incident report following a safety event and aspects of team and organisational culture that must be attended to if the learning from reporting is to be maximised for patient benefit.

  • The importance of creating an accurate, timely and comprehensive event report.
  • Best practice for rapid learning in Never Events.
